Jobing Description
Total Longterm Care is a non-profit PACE program that provides managed health care and supportive services designed to assist seniors who want an alternative to nursing home placement. With Total Longterm Care's assistance, participants can stay in their own homes and communities. The mission of Total Longterm Care is to enable frail individuals to live with dignity in their communities.

We are currently seeking an Enrollment Representative to join our new Pueblo team! Construction of our new Pueblo facility is underway. We anticipate the start date of this position to be December 1st.

POSITION SUMMARY:

Under the supervision of the Director of Enrollment, is responsible for the participant intake process including intake calls and screenings, providing information and education to prospective participants and/or callers, and scheduling home visits for the Assessment Specialist. This individual also enters initial referral into the participant tracking system and gathers data and follows-up on information needed for enrollment. Performs enrollments. Provides appropriate community referrals for persons not qualifying for the Total Longterm Care program. Educates potential referral sources regarding Total Longterm Care’s program and maintains contact with existing referral sources.

PHYSICAL DEMANDS AND WORKING CONDITIONS:
1. Physical Requirements - Requires full range of body motion including handling and lifting records and forms, use of telephone and computer to gather information. Requires standing, walking, pushing, bending, kneeling and reaching at arm’s length and overhead in office setting and in community periodically. Must have a valid Colorado Drivers license and a reliable mode of transportation. Must be flexible in relation to hours worked in order to meet the scheduling needs of prospective participants and their families. Requires the ability to lift/carry up to 30 pounds using appropriate body mechanics.

2. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate clearly and effectively over the telephone and comfortable in speaking in public to small or large groups. Good verbal and written communication skills with ability to maintain accurate records and to prepare clear and concise reports, correspondence and other written materials. Excellent customer service skills.

3. Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments while dealing with frail and confused participants. Subject to participants that may have the potential for verbal or physical aggression.

4. Environmental Conditions - May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a clinic and participants home environment. Subject to unpleasant odors and varied living situations of participants.

Skills / Requirements
QUALIFICATIONS:

MUST PASS A PRE-EMPLOYMENT PHYSICAL AND DRUG SCREEN.

Requires a B.A. or B.S. in Human Services or comparable education and experience. Two years of health care experience working with frail and/or chronically ill elderly population. Exceptional telephone and customer service skills. Ability to effectively and accurately represent the program to participants, families, outside agencies, and the general public. Knowledge of community resources available to seniors and their families. Skilled in identifying and recommending problem resolution. Skilled in establishing and maintaining effective working relationships with participants, co-workers, medical staff and the public. Knowledge of intake and sales practices. Good organizational skills, flexibility and initiative. Ability to maintain accurate records, compile data and prepare reports. Current Colorado Drivers License, proof of auto insurance and consistent and reliable usage of a motor vehicle. Ability to work independently with minimum supervision. Confidentiality required.
