Total Longterm Care, a non-profit organization, was established in 1991 as Colorado’s PACE provider. PACE (Program of All-inclusive Care for the Elderly) is an innovative system of care designed to meet the needs of nursing-home eligible individuals. By combining medical care, community-based home and health-care services, and day programs, PACE providers help these individuals stay in their own homes and communities and out of nursing homes. There are PACE programs throughout the United States.
Today, TLC has five metro area Day/Health Centers and a network of transportation to ensure that all participants have an easy ride to and from their homes. The program continues to grow; but the goal of the program, to help each individual in a personal way, remains constant.
The mission of Total Longterm Care is to enable frail individuals to live with dignity in their communities.

Total Longterm Care provides and coordinates all health care and in-home services for participants based on recommendations of the care team. Ongoing assessments determine what range of services meet an individual’s needs.
